Metrocare Services seeks a QMHP-Adult Intensive to provide psychosocial rehabilitation, crisis intervention, and case management for adults with serious mental illness. You will assess needs, develop recovery goals, coordinate with a multidisciplinary team, and ensure continuity of care from admission through discharge.

The role emphasizes collaboration, documentation, and supportive services to promote community reintegration and reduce hospitalizations within a trauma-informed framework.

Qualifications

• Bachelor’s in human services with 1–3 years MH/MR experience or Master’s with 0–2 years experience.
• Strong interpersonal and documentation skills.
• Ability to work within an interdisciplinary treatment team.

Responsibilities

• Provide psychosocial rehabilitation, emergency services/assessment, referral, linkage, and advocacy.
• Develop measurable recovery goals and plans with individuals.
• Coordinate care with an interdisciplinary treatment team and ensure service authorization.
• Monitor progress and adjust treatment plans; document outcomes in a timely manner.
• Support medication adherence and facilitate transportation and follow-up as needed.
• Participate in discharge planning and community reintegration; maintain contact post-discharge.
