Job summary

Grace Pointe Senior Care Community in Greeley, CO, is seeking PRN QMAPs to join the Assisted Living/Memory Care Department.

The role includes administering medications as ordered by the physician, under supervision, and supporting residents with daily activities, mobility, and meals.

Qualifications

• Must hold QMAP certification.
• Experience administering medications in assisted living/memory care is preferred.

Responsibilities

• Answer resident call lights and assist with needs.
• Follow service plan and assist residents with activities of daily living.
• Escort residents to activities and dining areas as needed.
• Deliver meals and assist with dining service as required.
• Maintain residents' rooms and common areas; manage cleanliness.
• Provide laundry/linen services per service plan.
• Document significant changes in residents’ status to nursing staff.
• Accurately prepare, administer, and document medications.
• Verify any meds brought by new residents with nursing leadership.
