Job summary

Experience Senior Living Team is seeking a dedicated Care Partner/QMAP for night shifts Sunday-Thursday in Fort Collins, CO. The role emphasizes compassionate personal care, ADLs support, and enabling participation in enrichment activities.

You will work with a team to maintain high standards of resident care, safety, and confidentiality while coordinating with management and other departments. This position requires dependable scheduling and a passion for senior living.

Qualifications

• High School Diploma or equivalent required.
• CNA/Home Health Aide preferred but not required.
• CPR or First Aid certification preferred.
• Ability to read and comprehend simple instructions and communicate effectively.

Responsibilities

• Assist residents with personal care and activities of daily living as assigned and per care plan.
• Perform daily housekeeping and laundry duties.
• Review 24 Hour Reports and shift notes; communicate changes to supervisor.
• Assist residents to and from life enrichment programs and activities.
• Maintain infection control practices and ensure resident safety.
• Respond to resident pendant/wearable calls promptly and communicate needs to Health & Wellness Director.
• Support residents’ independence and dignity while respecting rights.
