Job summary

Envision Unlimited is seeking a Qualified Intellectual Disabilities Professional (QIDP) to ensure members receive comprehensive case management in community living settings. You will coordinate, implement, and document individualized programs to promote independence and collaborate with guardians, staff, and healthcare providers.

Qualifications

• Bachelor's degree in Human Service field and at least 3 years in developmental disabilities.
• Must be a QIDP with verification letter.
• Excellent written and communication skills, plus computer and organizational skills.
• Proof of Direct Service Professional training and STOFHLA test.
• Valid Illinois driver's license and ability to travel for program needs.
• Criminal background check and Nurse Aide Registry clearance.

Responsibilities

• Develop and implement individualized, progressive programs for members in community living settings.
• Schedule, assign, document, and monitor member activities to meet goals.
• Develop implementation plans and complete ICAP and SLOF assessments.
• Ensure timely CIRAS, incident, and hospitalization reports in the electronic health record.
• Submit documentation to Human Rights Committee and Behavior Management Committee; attend meetings.
• Coordinate services and ensure implementation of recommended services.
• Attend psychiatric and medical appointments as needed.
• Maintain accurate member records and attendance reports per guidelines.
• Communicate with parents, guardians, and community agencies; document contacts.
• Collaborate with nursing and direct care staff on medical appointments and follow-up.
