Job summary

Avamere seeks a Full-Time QAPI Resource Registered Nurse to lead quality assurance initiatives across assisted living and memory care communities in the Northwest region. The role collaborates with facility leadership to analyze data, prevent negative trends, and support survey readiness.

You will conduct audits, mentor staff, and ensure proper documentation and EMR utilization while traveling regionally to facilities in Oregon, Washington, and Nevada as needed.

Qualifications

• Must possess a nursing degree from an accredited college or university.
• Must possess an active RN license in the state.
• BSN preferred and knowledge of CMS/state regulations.
• Experience in assisted living, memory care, or geriatric settings.
• Ability to design and deliver training and manage audits.

Responsibilities

• Lead QA and performance improvement projects across multiple communities.
• Conduct audits of care plans, MARs, documentation, and infection control.
• Maintain survey readiness and assist with state inspections.
• Mentor Wellness Directors and care staff regionally.
• Ensure EMR/EHR utilization and data analysis for trends.
