Job summary

Avamere seeks a QAPI Resource Registered Nurse to lead quality assurance and performance improvement across assisted living and memory care communities. You will partner with facility leadership to analyze data, drive regulatory compliance, and ensure survey readiness.

The role requires travel and hands-on mentoring of Wellness Directors and care staff, with emphasis on documentation, infection prevention, and CMS/state regulations.

Qualifications

• Must hold a nursing degree from an accredited college.
• Current, active RN licensure in Oregon; BSN preferred.
• Experience in ALF/Memory Care and CMS/state regulations.

Responsibilities

• Conduct audits of resident care plans, MARs, documentation, and infection control.
• Analyze facility data to identify negative clinical trends and risks.
• Design, implement, and monitor QA/PI projects across locations.
• Travel 50–80% to facilities in region (OR, WA, NV) and support survey readiness.
