Job summary

Avamere Living in Tigard, Oregon seeks a QAPI Resource Registered Nurse to lead quality assurance across multiple assisted living and memory care communities. This role collaborates with facility leadership to analyze data, prevent adverse trends, and ensure survey readiness.

Responsibilities include conducting audits, implementing improvement projects, mentoring staff, and maintaining EMR documentation. Travel up to 80% to regional facilities is required.

Qualifications

• Must possess a nursing degree from an accredited college or university.
• Must possess a current, unencumbered, active RN license in Oregon; BSN preferred.
• Experience in assisted living, Alzheimer/dementia, long-term care, geriatric rehab, sub-acute or critical care.
• Thorough knowledge of CMS and State Regulations for ALF/IL/MC facilities.
• Experience in electronic medical records documentation.
• Strong leadership and communication skills.
• Ability to design and deliver effective training programs.
• Knowledge of state and federal regulations and compliance requirements.
• Excellent organizational and time management skills.
• Must have and maintain an active CPR/BLS during employment.

Responsibilities

• Conduct audits of resident care plans, MARs, documentation, and infection control.
• Analyze facility data (falls, weight loss, pressure ulcers, infections) to identify trends.
• Design, implement, and monitor quality improvement projects across locations.
• Maintain survey readiness and assist during state/local surveys.
• Develop and oversee Plans of Correction for citations.
• Float between facilities to mentor Wellness Directors and care staff.
• Deliver staff training on documentation, regulations, infection prevention, and best practices.
• Partner with Administrators to review incidents and implement corrective actions.
• Ensure proper use of EMR/EHR systems.
