The QA & Performance Improvement Coordinator (RN, Quality) at Hartford HealthCare at Home is responsible for enhancing clinical documentation and outcomes through systematic, data-driven approaches. The role involves analyzing quality data related to patient care, safety, risk management, infection control, and customer satisfaction, and leading performance improvement initiatives based on audit results. Responsibilities include developing process improvement plans, conducting record reviews, data tracking and analysis, and serving as a regulatory resource for clinical staff. The coordinator promotes understanding and collaboration among team members to ensure best practices are utilized, participates in quality improvement activities, reviews compliance with regulations, and facilitates evidence-based clinical practices to optimize outcomes. The role also involves ensuring accurate documentation, developing interventions to improve quality scores, and supporting agency compliance and data submission requirements.
Qualifications include an Associate of Science Degree (required) and a Bachelor's degree in nursing or related fields (preferred). Candidates must have at least 3 years of certified home health experience, knowledge of federal and state regulations, experience with data analysis and quality reporting, and excellent communication skills. Travel within the region and possibly across the United States may be required. The position is full-time at a mid-senior level, with a focus on health care provider services within wellness and fitness industries.
