QA & Performance Improvement Coordinator (RN, Quality) - HomeCare

This role involves assisting with clinical documentation and outcomes improvement through systematic, data-driven approaches. Responsibilities include compiling trends, reporting quality data, developing performance improvement plans, conducting record reviews, analyzing data, and serving as a regulatory resource for clinical staff. The coordinator promotes communication and collaboration, participates in quality improvement activities, reviews assessments for compliance, ensures proper coding, develops interventions, and facilitates evidence-based practices to optimize clinical outcomes. They also ensure compliance with regulations, manage data submissions, and perform clinical audits to support quality care.

Qualifications: Associate of Science Degree required; Bachelor’s degree in nursing or related field preferred. Minimum of 3 years of certified home health experience required. Knowledge of federal and state regulations, data analysis, quality reporting, and performance measurement is essential. Licensure in Connecticut and ability to travel are required.
