The Quality Assurance & Performance Improvement (QAPI) Specialist supports agency efforts to improve clinical documentation and outcomes through systematic, data-driven approaches to safety and quality. Responsibilities include analyzing quality data, developing improvement plans, conducting record reviews, and serving as a regulatory resource for clinical staff. Success in this role requires a passion for quality care and knowledge sharing.

Key duties include:

  • Promoting understanding, communication, and collaboration within teams.
  • Developing and evaluating clinical quality improvement plans.
  • Participating in QAPI activities related to patient and employee incidents, infection tracking, and quality indicators such as OASIS and patient satisfaction.
  • Assessing clinical staff compliance with policies and standards.
  • Data analysis, reporting, and review of OASIS assessments for compliance.
  • Ensuring accurate ICD-10 coding related to patient conditions.
  • Creating interventions to improve quality scores and participating in related groups.
  • Reviewing corporate data and promoting culturally sensitive performance improvement plans.
  • Conducting clinical record audits and providing educational feedback.
  • Facilitating evidence-based practice adoption to optimize outcomes.
  • Ensuring organizational compliance and timely data submission to payors and regulators.

Qualifications:

  • Associate of Science Degree required.
  • Bachelor’s degree in nursing, PT, OT, SLP, MSW, Health Administration, or related field preferred.
  • At least 3 years of certified home health experience required.
  • Experience with Value-Based Purchasing, PDGM, CAHPS preferred.
  • CT Licensure in relevant field required.
  • Experience in data analysis, quality reporting, and performance measure development.
  • Strong communication skills and willingness to travel as needed.
  • Knowledge of federal and state regulations.
