Job description

QA & Performance Improvement Coordinator (RN, Quality) - HomeCare

This role involves assisting with agency efforts to improve clinical documentation and outcomes through systematic, data-driven approaches to safety and quality. Responsibilities include compiling trends, reporting quality data, developing performance improvement plans, conducting record reviews, analyzing data, and serving as a regulatory resource for clinical staff. The coordinator promotes collaboration, participates in quality improvement activities, reviews assessments for compliance, and ensures proper coding and documentation. They also develop interventions to improve quality scores, facilitate evidence-based practice, and ensure regulatory compliance.

Qualifications: Associate of Science Degree required; Bachelor's in Nursing or related field preferred. At least 3 years of certified home health experience is required, with knowledge of federal and state regulations, data analysis, and quality reporting. Licensure in Connecticut and experience in quality improvement methodologies are necessary. Strong communication skills and willingness to travel are also required.
