Job summary

Qualifications

• The candidate must have PharmD and PGY1 residency.
• PGY2 Ambulatory Care residency or equivalent experience preferred.
• Illinois licensure required at time of employment.
• Strong clinical knowledge base and communication skills.

Responsibilities

• Provide pharmaceutical care to patients with pulmonary diseases under a telemedicine model.
• Participate in assessment and management of asthma and COPD with a credentialed practitioner.
• Develop and implement evidence-based care plans and order related labs.
• Collaborate with nursing, physician, and pharmacy staff; support quality initiatives.
• Contribute to didactic lectures and precepting pharmacy students and residents.
