Job summary

SMG System Medical Group seeks an Advanced Practice Professional (APP) to deliver high-quality, patient-centered pulmonary care as part of a multidisciplinary team. The APP assesses, diagnoses, treats, and manages patients within scope, coordinating care with physicians, nurses, and other professionals.

The role supports residents and students, oversees discharge planning, and participates in education and program development.

Qualifications

• Graduate of a CCNE accredited nursing program with Master’s Degree in Nursing.
• National Certification by one of the recognized APRN Certification Centers.
• Licensure or eligibility for licensure as a professional RN and APRN.
• BLS Certification with Advanced Life Support training as required.

Responsibilities

• Manages comprehensive care for pulmonary patients in inpatient and ambulatory settings.
• Performs daily rounds, pre-procedure evaluations and integrated assessments.
• Orders tests or consults per guidelines and coordinates discharge planning as applicable.
• Communicates results of diagnostic tests and abnormal findings to clinic and hospital staff.
• Documents care plans and coordinates follow-up appointments and referrals.
