Job summary

PHMC is seeking a Nurse-Family Partnership Public Health Nurse Home Visitor in Philadelphia to provide comprehensive nursing services to first-time pregnant people and their families enrolled in the NFP program. The NHV conducts in-home visits, adheres to the NFP model, supports families to meet goals, and collaborates with NNCC and staff.

The position requires a Bachelor of Science in Nursing and PA licensure, with a flexible schedule and travel within Philadelphia.

Qualifications

• Bachelor's degree in Nursing or equivalent.
• Current Pennsylvania nursing licensure in good standing.
• Experience in public health, maternal-child health, or related field preferred.

Responsibilities

• Completes all required NFP/NNCC education and STAR Framework.
• Provides in-home and telehealth visits, up to 36–40 visits/month at full caseload.
• Builds and maintains a caseload of 25 active clients within 9–12 months.
• Develops therapeutic relationships using motivational interviewing and FAN.
• Schedules home visits with NFP supervisor every four months.
• Collaborates with outreach staff on referrals and community resources.
