Job summary

Qualifications

Responsibilities

• Provide home visits to pregnant women and their child up to age 2.
• Carry a caseload of up to 28 clients.
• Deliver education, support and referral resources to families.
• Complete all required NFP trainings and adhere to the NFP model.
