King County’s Public Health Department is seeking a Public Health Nurse (PHN) for the Nurse-Family Partnership (NFP) in the Kent Public Health clinic. The PHN supports pregnant clients through the child’s second birthday with home or community visits, building lives for families with ongoing supervision and team collaboration.

Ideal candidates hold a nursing degree and WA RN license, with BLS-CPR. Bilingual Spanish/English is a plus.

Qualifications

Bachelor's degree in nursing science or a graduate level nursing degree (MSN/DNP/PhD) from an accredited nursing school.
Current Washington State license as a Registered Nurse or multi-state license.
Documentation of current BLS-CPR for Health Care Providers.
Washington state driver's license.
Ability to pass background and credentialing.

Responsabilités

Obtain and document health history and make nursing diagnoses based on assessment.
Document electronically using EPIC and NFP database.
Provide public health nursing expertise across levels.
Link/refers client to other staff and community partners to ensure access and continuity of care.
Plan program activities, monitor local work plans and progress toward goals.
Participate in quality assurance activities to meet laws, regulations, policies and procedures.
Provide training and education to staff, clients, students and the community.
Offer birth control and pregnancy options counseling as needed.
Perform other duties as assigned.
