Job summary

Family First Health is seeking a part-time Registered Nurse for the Nurse Family Partnership program in York, PA. The RN will provide nursing, education and support to first-time mothers and families, with home visits across multiple sites.

The role requires an RN license and BSN, CPR and Narcan certification, and strong customer service and organizational skills. Travel between work sites and patient homes is expected as part of the role.

Qualifications

RN license and a BSN required.
Current CPR and Narcan certification.
Knowledge of Microsoft Office and clinical software for reporting.
Excellent customer service and communication skills.
Strong attention to detail and analytical abilities.
Ability to travel between work sites and conduct home visits.

Responsibilities

Provide nursing, education and support services to first-time mothers and families.
Develop therapeutic relationships and maintain boundaries during home visits.
Assess needs across 6 domains and plan interventions.
Adapt program guidelines to individual patient needs.
Refer to community resources and coordinate care.
Maintain timely documentation and schedule management.
