Qualifications

• Associate degree in nursing and one year of nursing experience plus completion of a PHN course within one year of hire and current Georgia RN license and CPR certification; or one year of experience at PH RN 1 (HCP190) or equivalent.

Responsibilities

• Provide comprehensive nursing case management for TB patients, including assessment, treatment monitoring, medication adherence, education, and therapy completion.
• Coordinate and monitor follow-up for patients with STDs and abnormal mammogram/Pap results, including referrals and treatment coordination.
• Deliver direct patient care per Georgia DPH nurse protocols, using clinical judgment to assess, treat, educate, and monitor.
• Perform assessments, develop care plans, monitor progress, coordinate referrals, and collaborate with physicians to ensure continuity of care.
• Educate patients and families on disease processes, treatment plans, medications, prevention, resources, and healthy lifestyle choices.
• Maintain accurate documentation in the EMR and reporting systems; ensure compliance with regulations and program standards.
• Collaborate with multidisciplinary teams and community partners to improve access to services and support initiatives.
• Participate in community outreach, health promotion, and public education activities.
• Engage in QA/QI activities including chart reviews, peer reviews, and protocol development.
• Participate in emergency preparedness planning and required certifications.
