• 1 year of experience engaging in case management activities such as enrolling customers in public benefits programs.
• Mastery of MS Office: Word, Excel, Outlook, and Teams.

Responsibilities

• Conduct intake interviews with clients to assess housing, food, healthcare, and personal finances needs.
• Receive information from staff and risk/needs assessment tools to determine essential needs.
• Serve as Certified Application Coordinator to submit electronic benefits applications to MassHealth within a designated organization.
• Access DTA Connect to submit electronic benefits applications to SNAP for clients.
• Assist eligible clients with SSI/SSDI applications and understand related eligibility requirements.
• Support veterans benefits applications and eligibility requirements.
• Engage in outreach to develop updated information about benefits programs.
• Conduct community outreach to identify benefit program leads for unmet needs.
• Cross-train in actuarial assessment tools used for risk/need and treatment planning.
