Los Angeles County Department of Mental Health (DMH) seeks Medical Case Workers to join Public Guardian. Work under supervision to assist in the provision of services to clients by performing clerical, caregiver, homemaker, and sub-professional duties.

Responsibilities include verifying entitlements (Social Security, VA, Medi-Cal, Medicare), maintaining benefits via redeterminations, ensuring proper spending and payments, and coordinating with vendors.

Qualifikationen

• Experience in case management or health/social services settings.
• Ability to understand health and social service programs applicable to the role.
• Strong organizational and communication skills.

Aufgaben

• Assist rep-payee participants in verifying and applying for entitlements (Social Security, VA, Medi-Cal, Medicare).
• Submit verification documents and complete annual redeterminations to maintain benefits.
• Ensure spending patterns maintain eligibility for public benefits, pensions, and other income.
• Provide basic case management and accompany clients to appointments to maintain benefits.
• Ensure payment for services, basics, and rent for participants.
• Set up payment recipients as vendors and verify their status.
• Ensure contracted provision of paid services.
• Provide personal and incidental funds to clients as appropriate.
• Pick up mail and checks as needs arise.
