Under close direction, you will join a multidisciplinary team in hospitals, clinics, or care facilities and help patients navigate housing, welfare, and clinical resources.

The role emphasizes interviewing, care planning, and referrals, with an initial period of up to 90 days under emergency order, potentially leading to permanent County employment.

Qualifications

• Bachelor's degree from an accredited college or university.
• Official transcripts or diploma must be attached at filing.
• Foreign degrees must be evaluated for equivalency by credential evaluation agencies.

Responsibilities

• Interview patients/clients, family members, and friends to obtain information affecting illness and recovery.
• Assist with environmental difficulties and arrange services (child care, foster homes, homemaker, attendant care).
• Know community, mental health, and welfare resources and programs.
• Participate in medical rounds and care team conferences to plan treatment.
• Assist in placing patients in board and care, intermediate, private facilities, or hospitals.
• Counsel patients in health topics and make referrals for therapeutic abortion, tubal ligation, or vasectomies.
