Job summary

Heluna Health in Los Angeles is seeking Medical Case Workers to support the Los Angeles County Department of Mental Health's Public Guardian program. You will assist clients in navigating benefits, complete documentation, and coordinate with health and social services staff to maintain eligibility.

Qualifications

• One year of experience as a case manager, or a bachelor’s degree in a health-related field.

Responsibilities

• Assist in verifying and applying for entitled benefits including Social Security, VA, Medi-Cal and Medicare.
• Submit verification documents and complete annual redeterminations to maintain benefits.
• Ensure proper spending patterns to maintain eligibility for public assistance benefits, pensions, and other income.
• Provide basic case management and accompany clients to appointments to maintain benefits.
• Set up payment recipients as vendors and verify their status.
• Ensure paid services are provided as contracted and provide personal funds to clients as appropriate.
• Pick up mail and checks according to client needs.
