County of Los Angeles Department of Mental Health seeks qualified candidates for emergency Medical Case Worker II roles to address homelessness. Under an emergency order, hires may be up to 90 days with potential for permanent employment and a performance-based assessment.

Under professional direction, the worker will join a multidisciplinary team to provide services in hospitals, clinics, and care facilities, delivering counseling, care coordination, and referrals to community resources.

Qualifications

• A Bachelor's degree from an accredited college or university and one year of professional casework experience interviewing, counseling, and assisting patients/clients with social problems.

Responsabilités

• Interviews patients, family members, and friends to obtain information affecting medical and mental health treatment.
• Uses environmental data to recommend services for maximum patient benefit.
• Arranges ongoing or post-operative home care and coordinates for nursing or extended care facilities.
• Assists patients and families with environmental difficulties and arranges homemaker or attendant care services.
• Keeps informed on community health resources and programs.
• Makes referrals to public and private health and social agencies to resolve problems.
