Job summary

The Franklin County Board of Commissioners is seeking a Case Manager to determine eligibility for TANF, Medicaid, Food Assistance and other public programs. You will interview clients in person, by phone, or in the field, gather documentation, authorize or deny participation, and coordinate services.

You will develop plans, assist clients to access benefits, monitor compliance, conduct case conferences, maintain records, and attend trainings.

Qualifications

• Associate degree in social work or human services with two years of relevant experience preferred.
• Bachelor s degree acceptable; related training/experience also considered.
• Knowledge of public assistance programs and eligibility requirements is expected.

Responsibilities

• Interview clients to assess needs and eligibility for TANF, Disability Assistance, Medicaid, Food Assistance, and other programs.
• Gather and document personal, financial and employment information and required verifications.
• Develop cooperative plans to meet client needs and assist with service access.
• Monitor participation requirements and adjust plans as needed; conduct case conferences as required.
• Maintain documentation, logs and reports and attend trainings; travel within the county as needed.
