Job summary

Evolve Therapy Services, LLC in West Saint Paul, MN is seeking a licensed Physical Therapy Assistant to support an in-house Skilled Nursing Facility on a PRN schedule. You will provide therapy under the supervision of the Physical Therapist and help implement treatment plans for residents.

The role includes screenings, group and individual therapy, documentation in NetHealth EMR, and collaboration with the care team to maintain high quality resident care.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
• Assist in developing treatment plans and goals with the Physical Therapist.
• Deliver individual and group treatment per the treatment plan.
• Document CMS-compliant notes in NetHealth EMR and coordinate with the care team.
