Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
• Assist in the evaluation under the direction of the Physical Therapist.
• Assist in identifying goals and treatment plans with the Physical Therapist.
• Deliver individual and group treatment as outlined in the treatment plan.
• Document and monitor resident progress in CMS-compliant NetHealth EMR records.
