Job summary

Evolve Therapy Services, LLC in Antioch, TN is hiring a Physical Therapy Assistant to support an in-house Skilled Nursing Facility. Full Time, Part Time or PRN positions available.

You will assist the Physical Therapist in planning and delivering therapy services to residents. Responsibilities include screenings, implementing treatment plans, group and individual sessions, documentation in NetHealth EMR, and education for residents and families.

Qualifications

• Associate degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education to maintain licensure.
• Must maintain current licensure and certification.

Responsibilities

• Provide PT services to residents under supervision of the Physical Therapist.
• Conduct screenings to determine need for intervention/treatment.
• Assist in evaluations and treatment planning under PT direction.
• Assist in establishing target goals and treatment plans with the PT.
• Deliver individual and group therapy including strength, mobility, balance, ROM and modalities.
• Monitor resident response and adjust therapy per PT guidance.
• Document compliant and medically necessary activities in NetHealth EMR as CMS requires.
• Coordinate resident schedules with PT and care team.
• Educate residents, families and staff on therapy techniques and equipment.
