Job summary

Evolve Therapy Services is hiring a Physical Therapy Assistant for an in-house Skilled Nursing Facility in Memphis, TN. The role supports therapy services under the supervision of a Physical Therapist and includes screenings, treatment planning, and resident education.

Full Time or PRN options are available. The position emphasizes quality resident care, adherence to CMS guidelines, and proactive collaboration with the care team.

Qualifications

• Associates degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
• Assist the Physical Therapist in identifying goals and treatment plans.
• Conduct individual and group treatment according to the plan.
• Assist in selecting techniques/media and adapt as indicated by resident performance.
• Monitor residents’ response and modify as needed.
