Job summary

Evolve Therapy Services is seeking a compassionate Physical Therapy Assistant for an in-house Skilled Nursing Facility located in Antioch, TN. The role supports the Physical Therapist in delivering services, conducting screenings, and developing treatment plans within CMS guidelines.

Positions available as Full Time, Part Time or PRN. Requires an Associate's degree in physical therapy and an active PTA license. NetHealth EMR experience preferred; benefits available for full-time status.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active PTA license in state(s) of practice is required.
• Ongoing continuing education to maintain licensure.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents to determine need for intervention/treatment.
• Assist in evaluations and treatment planning with the Physical Therapist.
• Develop and modify resident schedules with the PT and care team.
• Educate residents, families, and staff on therapies and safety.
• Document compliant and medically necessary CMS-ready notes in NetHealth EMR.
