Job summary

Evolve Therapy Services, LLC is hiring a licensed Physical Therapy Assistant for an in-house Skilled Nursing Facility in Antioch, TN. You will work under the Physical Therapist to implement treatment plans and support resident mobility and function.

The role requires an Associate’s degree in physical therapy, active PTA licensure in the state, and commitment to ongoing education. Full Time, Part Time, or PRN schedules may be available, with competitive benefits.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Includes ongoing continuing education and licensure maintenance.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents to determine intervention needs.
• Assist in developing targeted goals and treatment plans with the PT.
• Deliver individual and group therapy sessions per the plan.
• Use approved techniques and adapt activities as needed.
• Monitor responses to treatment and report changes to the PT.
• Assist in scheduling in coordination with the PT and team.
• Prepare documentation compliant with CMS expectations in NetHealth EMR.
