Job summary

Evolve Therapy Services is seeking a Physical Therapy Assistant for an in-house Skilled Nursing Facility partner in Antioch, TN. The position offers full-time, part-time, or PRN schedules to support resident care under the supervision of a Physical Therapist.

The role emphasizes screenings, treatment planning, gait and mobility training, documentation in NetHealth EMR, and collaboration with the care team to maintain high-quality patient outcomes.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Must have an active Physical Therapy Assistant license in state(s) of practice.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
• Identify indications for evaluations at appropriate intervals and assist in the evaluation under the direction of the Physical Therapist.
• Assist the Physical Therapist in identifying goals and treatment plans.
• Conduct both individual and group treatment as assigned by the Physical Therapist and outlined in the treatment plan.
• Assist in selecting PT techniques/media and sequence of activities with guidance from the PT.
• Monitor residents’ response to intervention and consult with the PT when modification is needed.
• Assist in establishing and modifying individual resident schedules with the PT and team members.
• Complete compliant documentation from CMS expectations in NetHealth EMR systems.
