Job summary

Elara Caring in Indiana seeks a Licensed Physical Therapist Assistant to provide hands-on rehabilitation in patients' homes. You will implement the plan developed by the supervising Physical Therapist, monitor progress, and educate patients and families on home therapy practices.

Responsibilities include guiding mobility goals, safely using assistive devices, and ensuring care aligns with physician orders and state guidelines. Travel between patient homes is required.

Qualifications

• Current, unrestricted PTA license valid for the state of work.
• Graduate of an accredited Physical Therapy Assistant program.
• 6+ months experience as a PTA preferred.
• Ability to frequently lift, push, pull, and support up to 50 pounds.
• Must possess a valid driver’s license or reliable transportation to travel between patient homes and assigned locations.

Responsibilities

• Provide physical therapy services according to the treatment plan established by the Physical Therapist.
• Help patients work toward therapy goals that improve mobility, function, and independence.
• Monitor patient progress and report changes to the supervising Physical Therapist promptly.
• Educate patients and caregivers on home therapy programs and techniques.
• Teach patients and families how to safely use assistive devices (canes, walkers, wheelchairs, prosthetics, orthotics).
• Deliver safe, high-quality therapy services in accordance with physician orders and state guidelines.
