Job summary

LiveWell Homecare Agency, LLC is seeking a Licensed Physical Therapy Assistant to support patient care under a supervising physical therapist. The role involves executing prescribed interventions, educating patients and families, and documenting treatment activities consistent with policy.

You will coordinate with the PT on progress and adjustments while promoting growth and continued clinical development within a homecare setting.

Qualifications

• Associate’s Degree in Physical Therapy Assistance is required.
• Licensed PTA licensure is required.
• BLS certification is required.
• Board eligibility and continuing education, especially for acute stroke care, are expected.

Responsibilities

• Implement patient care plans under the supervision of a physical therapist.
• Educate patients, families, and healthcare providers about treatment.
• Document treatment activities in accordance with policy.
• Coordinate with the PT on patient progress and treatment adjustments.
