Job summary

Civita Care Center at Meadowbrook in Granby, CT is seeking a licensed Physical Therapist Assistant (PTA) to join its rehabilitation team. This full-time position supports residents in a skilled nursing and rehabilitation setting, helping them regain strength, mobility, and daily independence.

Qualifications

• Active Physical Therapist Assistant license required.
• Strong communication and interpersonal skills.

Responsibilities

• Carry out physical therapy treatment programs under the supervision of a licensed Physical Therapist
• Assist residents with therapeutic exercises, gait training, transfer training, balance activities, and mobility interventions
• Support individualized treatment plans focused on improving functional outcomes and resident independence
• Monitor resident participation and response to treatment sessions
• Communicate progress and observations to the supervising Physical Therapist
• Complete accurate and timely clinical documentation
