Job summary

Ayden Healthcare of Oregon is seeking a dedicated Physical Therapy Assistant (PTA) to join our skilled nursing facility team as a PRN contributor. You will collaborate with licensed physical therapists to deliver high-quality rehabilitation and support patient mobility and independence.

The ideal candidate will have an associate degree in PTA, current state licensure, and a compassionate, team-oriented approach to patient care, discharge planning, and family education.

Qualifications

• Requires PTA licensure/certification in the relevant state.
• Experience in outpatient clinics, SNFs or home health preferred.
• Strong communication with patients, families and healthcare team.
• Ability to work in a multidisciplinary team with compassion.
• Familiarity with discharge planning and individualized care plans.

Responsibilities

• Assist physical therapists in implementing treatment plans.
• Conduct patient assessments and monitor progress.
• Support patients in performing therapeutic exercises to improve strength, flexibility and mobility.
• Educate patients and families about treatment plans and home exercises.
• Document patient care activities and maintain records.
• Collaborate with healthcare team on discharge planning strategies.
• Maintain a clean and safe working environment per health and safety protocols.
