The ideal candidate holds an associate degree in PTA, is licensed, and has experience in settings like SNFs or outpatient clinics. Strong communication and teamwork are essential to support patient care and discharge planning.

Qualifications

• Associate degree from an accredited program.
• Current state licensure or certification as a PTA.
• Experience in outpatient clinics, SNFs, or home health preferred.
• Strong communication with patients, families and team members.
• Ability to work in a multidisciplinary team with empathy.

Responsibilities

• Assist physical therapists in implementing treatment plans.
• Conduct patient assessments and monitor progress.
• Support patients performing therapeutic exercises to improve strength, flexibility, and mobility.
• Educate patients and families about treatment plans and home exercises.
• Document patient care activities and maintain comprehensive records.
• Collaborate on discharge planning for smooth transitions.
• Maintain a clean and safe working environment.
