Job summary

Ayden Healthcare of Wauseon is currently seeking a Physical Therapy Assistant (PTA) PRN to join our skilled nursing facility team. You will work closely with licensed physical therapists to deliver high-quality patient care and support rehabilitation goals.

The role involves implementing treatment plans, conducting assessments, guiding exercises, educating patients and families, documenting care, and assisting with discharge planning in a collaborative multidisciplinary setting.

Qualifications

• Requires an Associate degree in Physical Therapy Assisting from an accredited program.
• Current state licensure or certification as a PTA.
• Experience in outpatient clinics, SNF, or home health preferred.
• Knowledge of rehabilitation practices for diverse patient populations.
• Excellent communication and teamwork abilities.

Responsibilities

• Assist licensed physical therapists in implementing treatment plans.
• Assess patients and monitor progress to ensure effective outcomes.
• Guide patients through therapeutic exercises to improve strength and mobility.
• Educate patients and families on home exercise programs.
• Document care activities and maintain patient records.
• Collaborate with the care team on discharge planning.
• Maintain a clean, safe clinical environment and comply with safety protocols.
