Job summary

Evolve Therapy Services, LLC seeks a licensed Physical Therapy Assistant to provide in-house therapy services at a Skilled Nursing Facility in Ponca City, OK. You will work under the supervision of a Physical Therapist, perform screenings, and contribute to treatment planning and execution.

Qualifications

• Associate's degree in Physical Therapy from an accredited program.
• Active Physical Therapy Assistant license in state of practice.
• Ongoing continuing education to maintain licensure/certification.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
• Assist in identifying target goals and treatment plans.
• Conduct individual and group treatment as outlined in the plan.
• Monitor residents' response to intervention and adjust as directed.
• Maintain resident schedules with the PT and team members.
• Train residents and staff in equipment use and safety.
• Complete documentation per CMS expectations in NetHealth EMR.

Education

Associate's degree in physical therapy

PTA license (state-specific)
