Job summary

Stern Therapy Consultants is seeking a PRN Physical Therapist Assistant to join Bedford Hills Center in Bedford, NH. The role provides hands-on treatment and helps maintain continuity of rehabilitation services in a skilled nursing setting.

You'll supervise under a Physical Therapist, guide residents through strengthening and balance exercises, document clearly, and collaborate with nursing and rehab peers.

Qualifications

• Active New Hampshire Physical Therapist Assistant license.
• Ability to provide individualized treatment within an established plan of care.
• Attentive observation, effective communication, and accurate documentation.
• New graduates are welcome to apply.

Responsibilities

• Carry out the supervising Physical Therapist's treatment plan, matching prescribed activities to residents' abilities and tolerance.
• Coach residents through strengthening, balance, and endurance exercises.
• Provide practice with walking, getting out of bed, and transferring safely between surfaces.
• Reinforce proper use of prescribed mobility devices and follow resident-specific precautions.
• Keep the PT informed of functional changes, treatment responses, and barriers to participation.
• Complete clear treatment notes and coordinate with nursing and rehabilitation colleagues on daily mobility needs.
