Stern Therapy Consultants is hiring a PRN Physical Therapist at Bedford Hills Center in Bedford, NH. You will evaluate residents, develop plans of care, and provide mobility-focused treatment to help improve functional independence.

Qualifications

• Active New Hampshire Physical Therapist license is required.
• Strong evaluation, treatment planning, and clinical reasoning abilities.
• Clear communication and precise documentation are essential.
• Post-acute rehab or skilled nursing experience is a plus; new graduates welcome.

Responsibilities

• Evaluate residents' strength, balance, ROM, and daily movement.
• Develop individualized plans of care with measurable goals for mobility.
• Provide therapeutic exercise, gait training, and mobility interventions.
• Assess assistive devices needs and educate residents and caregivers.
• Reassess, adjust treatment, and supervise PTA services.
• Maintain timely clinical documentation and collaborate on discharge planning.
