Job summary

Stern Therapy Consultants is seeking a PRN Physical Therapist Assistant for The Elms Center in Milford, NH. The role provides skilled nursing rehabilitation under the supervision of a Physical Therapist, with a pay rate of $35–$40/hour and PRN scheduling.

Responsibilities include executing plan-of-care treatments, aiding mobility, documenting sessions, and communicating progress to the supervising PT and care team. This position welcomes new graduates and offers a team-focused work environment.

Qualifications

• Active New Hampshire Physical Therapist Assistant license.
• Experience in skilled nursing is helpful; new graduates are welcome to apply.
• Ability to deliver individualized treatment within an established plan of care.

Responsibilities

• Provide treatment established in the PT's plan of care for residents recovering from illness, surgery, or injury.
• Facilitate exercises and movement activities that support strength, endurance, and balance.
• Practice walking, repositioning, and transfers with residents using prescribed mobility devices.
• Follow individual precautions and monitor comfort, fatigue, and tolerance throughout each session.
• Share observations with the supervising PT to support treatment decisions and continuity of care.
• Record services accurately and reinforce mobility recommendations with residents, caregivers, and nursing staff.
