Job summary

Fountain Inn Post Acute is seeking a compassionate Physical Therapist Assistant to join our rehabilitation team on a PRN basis. Working under the supervision of a licensed Physical Therapist, you will help residents improve mobility, strength, balance, and overall functional independence while contributing to a positive rehabilitation experience.

Qualifications

• Associate degree from an accredited Physical Therapist Assistant program.
• Current South Carolina PTA license in good standing.
• Long-term care or skilled nursing experience preferred.

Responsibilities

• Implement individualized treatment plans established by the Physical Therapist.
• Provide skilled physical therapy interventions focused on mobility, balance, strength, endurance, and functional independence.
• Monitor resident progress and communicate updates to the supervising therapist.
• Complete accurate and timely treatment documentation.
• Assist with patient and family education regarding therapy goals and functional activities.
• Participate in interdisciplinary team meetings and care planning as needed.
