Job summary

Fountain Inn Post Acute, a 66-bed skilled nursing facility in Fountain Inn, SC, seeks a compassionate Physical Therapist Assistant (PTA) to join the rehab team on a PRN basis. You will assist the licensed Physical Therapist to improve mobility, balance, and independence, while documenting care and educating residents and families.

Requirements include an Associate degree and SC PTA license; LTC experience preferred. PCC and Casamba experience are a plus.

Qualifications

• Associate degree from an accredited PTA program.
• Active South Carolina PTA license in good standing.
• Long-term care or skilled nursing experience preferred.
• Strong communication and interpersonal skills.
• Ability to collaborate with therapists, nursing staff, residents and families.

Responsibilities

• Implement treatment plans under the supervising PT.
• Provide PT interventions focused on mobility, balance, strength.
• Monitor resident progress and report updates to the supervising therapist.
• Complete accurate treatment documentation.
• Educate patients and families on therapy goals and activities.
• Participate in interdisciplinary team meetings and care planning.
