Job summary

Evolve Therapy Services is seeking a compassionate Physical Therapy Assistant for an in-house Skilled Nursing Facility in Olivia, MN. You will provide therapy services under the direct supervision of the Physical Therapist to support resident care.

Responsibilities include screenings, treatment planning, goal setting and documentation in NetHealth EMR, with a focus on teamwork with residents, families and staff. Full-time offers include a $5,000 sign-on bonus and tuition reimbursement.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Must have an active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
• Identify indications for evaluations and assist in the evaluation under the direction of the Physical Therapist.
• Assist the Physical Therapist in identifying targets and goals within the treatment plan.
• Conduct individual and group treatment focusing on strength, mobility, balance, ROM and modalities.
• Maintain compliant documentation in NetHealth EMR and collaborate with the care team.
