Job summary

Evolve Therapy Services, LLC is seeking a Physical Therapist for an in-house Skilled Nursing Facility. The role is full-time with a $5,000 sign-on and tuition reimbursement, plus part-time and PRN options.

You will plan, organize, and direct PT services to ensure high-quality resident care. Responsibilities include evaluating residents, developing treatment plans, documenting services, and collaborating with an interdisciplinary team in Olivia, MN.

Qualifications

• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents per established standards and procedures.
• Document therapy services accurately for CMS and billing in NetHealth.
• Evaluate residents and develop short- and long-term treatment goals.
• Perform screenings to determine need for intervention and treatment.
• Create and communicate treatment plans to residents, families, and the care team.
• Ensure compliance with local, state, and federal regulations in therapy services.
