Job summary

Evolve Therapy Services, LLC seeks a dedicated Physical Therapist to provide in-house services at our partner Skilled Nursing Facility in Olivia, MN. You will evaluate residents, plan treatment, document care, and collaborate with an interdisciplinary team to ensure high-quality resident outcomes.

We offer full-time, part-time, or PRN roles with competitive pay, a $5,000 sign-on bonus, tuition reimbursement, and benefits including medical/dental/vision.

Qualifications

• Bachelor of Science in Physical Therapy from an accredited program
• Active Physical Therapy license in state(s) of practice
• Ongoing continuing education and professional development to maintain licensure and certification
• Maintain licensure and certification as required by state and CMS guidelines

Responsibilities

• Provide physical therapy services to residents per standards and procedures
• Document and bill for PT services following CMS expectations
• Evaluate residents and develop treatment plans with short/long-term goals
• Communicate evaluation findings and plans to residents, families, and interdisciplinary team
• Complete compliant, medically necessary documentation in NetHealth EMR
