Job summary

Evolve Therapy Services, LLC in Olivia, MN seeks a compassionate Physical Therapy Assistant to provide in-house therapy under the supervision of a Physical Therapist. You will conduct screenings, assist in evaluations, help develop treatment plans, and deliver individual and group therapies focused on strength, mobility, balance, and gait.

You will help schedule residents, document outcomes in NetHealth EMR, and participate in quality assurance.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
• Identify indications for evaluations and assist in evaluating under the direction of the Physical Therapist.
• Assist in establishing and modifying individual resident schedules with the Physical Therapist and other team members.
• Train resident and staff in the use of therapy equipment.
• Document compliant and medically necessary data in NetHealth EMR.
