Evolve Therapy Services is hiring a Physical Therapy Assistant for an in-house Skilled Nursing Facility in Olivia, MN. The role supports the Physical Therapist and helps develop and implement therapy plans for residents.

Full-time with a $5,000 sign-on bonus and tuition reimbursement; part-time or PRN options are also available. Requires an active PTA license and an associate degree, with commitment to ongoing professional development.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Must have an active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
• Identify indications for evaluations and assist in developing treatment plans with goals.
• Assist in establishing and modifying individual resident schedules with the Physical Therapist and team.
• Train residents, families and staff in the use of therapy equipment.
• Document compliant and medically necessary CMS-related documentation in NetHealth EMR.
