Job summary

Evolve Therapy Services is seeking a Physical Therapy Assistant for an in-house Skilled Nursing Facility in North Platte, NE. This full-time role supports the Physical Therapist in delivery of therapy services and patient care on an on-site basis.

The PTA must hold an active license, have an associate degree, and participate in ongoing education. Competitive benefits and potential growth opportunities are offered.

Qualifications

• Associate degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Commitment to ongoing continuing education and licensure maintenance.

Responsibilities

• Provide physical therapy services under the direct supervision of a Physical Therapist.
• Conduct screenings of residents to determine need for intervention.
• Assist in evaluations and treatment planning with the Physical Therapist.
• Develop and modify treatment plans with the PT.
• Deliver individual and group therapy sessions as outlined in the plan.
• Assist with CMS-compliant documentation in NetHealth EMR.
• Participate in resident, family, and staff education.
• Maintain schedules and maximize productivity during shifts.
• Support Quality Assurance activities as required.
