Continuum Rehab Group is seeking a Physical Therapy Assistant to join our in-house therapy team serving Skilled Nursing/LTAC facilities in Rockwall, Texas. The role supports compassionate patient care, clinical growth, and a collaborative, clinically driven team environment.

Qualifications

• Licensed Physical Therapy Assistant required.
• Open to therapists at all experience levels.
• Provide rehabilitative, skilled and medically necessary treatment.
• Utilize evidence-based practice to support interventions.
• Communicate with supervising Physical Therapist to adjust treatments.

Responsibilities

• Uses evidence-based practice to support clinical interventions.
• Provides rehabilitative, skilled and medically necessary treatment under PT supervision.
• Provides skilled therapy services to patients needing rehabilitation.
• Assesses and documents patient progress; communicates with supervising PT.
• Provides counseling to patients and families about physical disorders.
• Contributes to discharge summaries of services.
